Tanzania Health Surveillance Form

The Tanzania Health SurveillanceForm is an obligatory short questionnaire needed for all travelers entering Tanzania. It is a formal document that must be completed to confirm that a person is not suffering from any symptoms of COVID-19 virus or any other contagious illness.

Health SurveillanceForm was introduced by Tanzanian government in order to minimize the COVID-19 pandemic outbreak in the country and ensure the safer environment both for international travelers and citizens of Tanzania.

The health declaration is mandatory to be filled out by every traveler visiting Tanzania within 24 hours before planned entry to the country in order to assure that the data will be as relevant as possible.

Those who plan to visit Tanzania should follow a quick online application process that involves providing some personal, travel, and health-related information to receive confirmation of a health form completion via email. The Health SurveillanceForm is necessary to enter Tanzania and will be checked upon arrival.

Important! Currently, from May 1, 2022, and until official notice is issued, the Health Surveillance System is suspended, as it is undergoing maintenance to improve its efficiency in detecting and notifying ill passengers.

COVID-19 restrictions in Tanzania

International travel to Tanzania is now open, but on arrival, all passengers must take a test quarantine until they get a negative result. A traveler can omit this process if they have: a COVID-19 vaccine certificate showing full vaccination; a negative COVID-19 NAAT or RT-PCR test taken within 72 hours before departure; or a recovery certificate issued between 10 and 180 days before arrival.

The new mandatory measure involves completing an online Health Surveillance Form. Those who plan to visit Zanzibar also need to fill out another Health Form made specifically for Zanzibar. Both forms should be completed 24 hours before arrival in Tanzania.

Passengers from countries defined as high risk in terms of reported coronavirus cases must also take antigen testing at own their expense. If the result is positive, they will have to undergo a 14-day quarantine until another testing will show a negative result.

All COVID-19 test requirements do not apply to children aged five years or younger.
