Portugal Passenger Locator Form (PLF) is an online health declaration that must be completed by all incoming visitors to Portugal within 4 days of their arrival.

How to complete the Portugal Passenger Locator Form

To start the application process locate the left-hand side panel and click on the 'Apply Online' button. Next, complete the process following these steps:

  1. Provide the details of your flight to Portugal, including the flight and seat number, name of the airline, date and time of the planned arrival, country of departure
  2. Select the exact destination you'll be arriving at (Lisboa/Porto/Faro/Tires)
  3. Put in your personal details, such as your name, date of birth, gender, address
  4. Provide the details of your passport
  5. Put in the address of the stay in Portugal
  6. Provide your contact details (email address and phone number)
  7. Put in the emergency contact information: name and phone number of the person that can be reached out for you for the next 15 days of your arrival in case of any emergency

After entering the required information into the online PLF Portugal application form, go through the provided information again to check it for correctness. If all details given are error-free, submit your application for approval and cover the service fee.

What happens after applying

Travelers who have submitted their Portugal Passenger Locator Forms will receive a confirmation of PLF completion via email along with their QR codes.

The confirmation arrives in PDF form and can be easily downloaded and printed or saved as an electronic copy.

Portugal border authorities check if each traveler has the confirmation of a completed Passenger Locator Card, either in electronic or paper form.

Covid-19 entry requirements in Portugal

Portugal introduced a variety of entry requirements that may differ between mainland Portugal, Azores, and Madeira, as well as based on the traveler's country of origin and/or vaccination status.

To enter the mainland of Portugal, all travelers aged 12 and over must have either:

  • an RT-PCR test taken within 72 hours of arrival
  • an antigen test taken within 48 hours of arrival
  • a valid test or recovery certificate

The above requirement applies to all travelers despite their vaccination status.

The currently applicable rules within Portugal include wearing face masks when in closed spaces, social distancing, hand washing, reduction in the capacity of commercial establishments, and the need to present an EU Certificate/negative test/or recovery certificate to enter bars, restaurants, gyms, or attend events.
