The Health Declaration Form or Passenger Locator Form (PLF) is a brief questionnaire, which provides the country's government with the essential health-related information about every person entering/leaving its territory.

The locator forms have been made obligatory because of the ongoing Covid-19 pandemic, serving as one of the safety measures that help the Dutch Government in minimizing the spread of infections and keeping under control any possible Covid-19 cases.

Travelers must answer a few questions relating to their health state and provide their flight details and basic personal information to complete the Travel Public Health Form for the Netherlands. The form may be easily filled out online before the planned trip using any working device with an internet connection.

How to fill the Netherlands Health Declaration?

To fill the Netherlands Health Declaration, travelers must follow a few simple steps. The health screening form can be completed entirely online, from home, or any other place using a phone, laptop, PC, or tablet.

Anyone who wishes to visit the Netherlands should fill out their Health Declaration online by:

  1. Clicking on the 'Apply Online' button - each applicant will then be forwarded to a health form to fill. The form can be completed within minutes by answering a few simple questions.
  2. Covering the issuing fee using any of the several available online payment methods.
  3. Waiting for a confirmation email to arrive.

Important! Travelers must use an active email address when applying for their Health Form to be able to receive the PDF confirmation without any issues.

Information to provide in the health form

The Dutch health declaration form can be divided into three main parts. The first part of the form contains three yes or no questions.

Travelers must answer if they have had any Covid-19 symptoms or have been diagnosed with Covid-19 or pneumonia, whether or not they have been in contact with someone possibly infected (within the last 10 days), and check if they have experienced any of the following symptoms within the previous 24 hours:

  • fever temperature
  • coughing
  • runny nose
  • sore throat
  • shortness of breath

Travelers who answer yes to any of the above-mentioned questions will not be allowed to board their flight to the Netherlands.

The second part of the Passenger Locator Form requires travelers to provide information relating to their flight, such as their flight number, their seat number. As well as some basic personal details, such as the traveler's surname and date of birth.

The third and last part is simply the date and time of filling the Health Declaration for the Netherlands completed with the traveler's signature.

Who needs to fill the Netherlands Health Declaration Form?

Anyone aged 13 or over must have a separate filled out Health Declaration when visiting the Netherlands by air. The PLF form requirement applies to all air travelers, including those who transit through the country.

Children or anyone who cannot fill the health form on their own should be included in their parents/guardians form.

Travelers who visit the Netherlands from within the EU/Schengen Area will only need a filled-out health declaration form (when going by air) if their country of departure is currently considered safe.

Coronavirus Entry Pass in the Netherlands

Starting from the 25th of September, anyone staying in the Netherlands must present a valid Coronavirus Entry Pass to:

  • attend any commercial events
  • go to the cinema
  • eat at the restaurant (indoors)
  • be a spectator of any sporting event
  • go to the casino

A Coronavirus Entry Pass is a health certificate that assures the Dutch Government that the spread of Covid-19 cases will be kept under control.

The Coronavirus Entry Pass is granted to anyone who has either proof of full vaccination, a negative Covid-19 test result taken within 24 hours of entering the selected area, or proof of being recovered from Covid-19.
