The Health Declaration Form or Passenger Locator Form (PLF) is a brief questionnaire, which provides the country's government with the essential health-related information about every person entering/leaving its territory.
The locator forms have been made obligatory because of the ongoing Covid-19 pandemic, serving as one of the safety measures that help the Dutch Government in minimizing the spread of infections and keeping under control any possible Covid-19 cases.

The Netherlands, as many countries now, has introduced a variety of different rules and regulations applicable to anyone visiting or residing within the country.
One of the recently introduced changes is the need for every air traveler to fill the Health Form before arrival. Filled forms will be checked when boarding one's flight, as well as upon arrival.
The Health Form is obligatory for any traveler aged 13 and over visiting the Netherlands via air. The form can be completed entirely online before the planned trip.
Quarantine and Covid-19 testing rules depend on a few factors. The Netherlands has divided foreign countries into safe and high-risk destinations, which helps define the applicable entry rules.
Moreover, travelers should take into account that different rules may apply for travelers with completed vaccinations and those arriving from outside the EU/Schengen Area.
There is a 10-day quarantine rule for travelers arriving from very high-risk areas; however, that period can be shortened by taking a Covid-19 test on the 5th day of self-isolation. What's more, travelers who have been fully vaccinated can omit the quarantine requirement when arriving from a high-risk country since the 22nd of September.
A negative Covid-19 test result or proof of vaccination is required for travelers arriving from outside the EU/Schengen Area that is considered to be a safe country upon departing.
Travelers who visit the Netherlands from within the EU/Schengen Area will only need a filled-out health declaration form (when going by air) if their country of departure is currently considered safe.
Starting from the 25th of September, anyone staying in the Netherlands must present a valid Coronavirus Entry Pass to:
A Coronavirus Entry Pass is a health certificate that assures the Dutch Government that the spread of Covid-19 cases will be kept under control.
The Coronavirus Entry Pass is granted to anyone who has either proof of full vaccination, a negative Covid-19 test result taken within 24 hours of entering the selected area, or proof of being recovered from Covid-19.
The Health Declaration for the Netherlands is a health screening form that serves as an obligatory document for anyone aged 13 and over entering or leaving the Netherlands.

The Dutch Health Declaration can be easily filled out online using any working device connected to the internet. The form should be filled out within 24 hours of arriving in the Netherlands and later on presented, either in electronic or paper form during the travel.
There are numerous Covid-19 rules applicable for Dutch visitors and citizens. The entry regulations, as well as the current restrictions within the country, depend on the person's vaccination status, as well as the country of arrival.
If one wishes to eat at an indoor restaurant, go to the cinema, attend a sporting or commercial event, go to a casino, they must have a Coronavirus Entry Pass. The entry pass can be given to anyone who has proof of complete vaccination, has taken a Covid-19 test within 24 hours of entry (that came back negative), a proof of recovery.
