– Birth certificate
– Authorization for issuance of Brazilian visas (signed by both parents)
– Passport/ID card of both parents****If only one parent is listed on the minor's birth certificate, only that parent's signature is required. If one parent has sole custody over a minor applicant, it is necessary to present a notarized copy of the court order awarding sole custody and exclusive parental rights. In case of one parent's death, a death certificate is required.
NOTE: For children of Brazilian parent(s), you need a signed document called Declaração de Não-Cidadania (Statement of Non-Citizenship) declaring that the applicant is not a Brazilian citizen.
